Understanding Hamstring Repair

A hamstring repair is a surgical procedure required when one or more of the three hamstring muscles tear away from the bone (the ischial tuberosity) in the pelvis. Often referred to as a tendon avulsion, this injury typically occurs during high-speed movements or sudden forceful stretching of the muscle.

Surgery is often the necessary path when the gap between the tendon and the bone is significant, as the muscle cannot heal back to its original strength through natural repair alone. Patients often experience severe weakness in the back of the leg, bruising, and significant limitations in hip mobility and walking ability prior to starting their rehabilitation journey.

Your Rehabilitation Timeline

Early Protection

PHASE 1

The immediate goal is to protect the surgical repair. During this initial period, we focus on pain management, wound healing, and gentle movement within safe limits, often using a brace or crutches to manage loading.

Gentle Activation

PHASE 2

As the tendon begins to knit, we introduce low-level activation exercises. This phase shifts toward restoring normal walking patterns and waking up the hamstring muscles without overstressing the repair site.

Strength & Foundation

PHASE 3

This is where the real work begins. We progressively increase resistance to build muscle volume and endurance. We monitor your capacity closely, ensuring the hamstring can handle higher loads needed for daily life and recreation.

Power & Return to Sport

PHASE 4

The final phase bridges the gap to full activity. We focus on explosive power, running mechanics, and sport-specific drills. Our goal is to ensure you don’t just return, but return stronger and more resilient than before.

The DJB Approach

At DJB Physiotherapy, we personalise hamstring repair rehab through a phased, data-informed strategy. We move beyond general templates, focusing on precise exercise progression and milestone-based monitoring. Our approach combines clinical expertise with clear, continuous communication, ensuring every step of your recovery is measured and managed to bridge the gap between surgery and peak performance.

Louise's Path to Performance

Louise who is regular gym member at Ady Watts came to the attention of DJB physio following a complex hamstring repair surgery. Lou began her journey with us wearing a restrictive post-operative brace and facing significant mobility limitations. Initially managing her symptoms and post op restrictions looking to regain rom and then strength. Her recovery required a meticulously phased approach, transitioning from early tissue protection to active loading. To bridge the gap between clinical healing and her return to high-intensity training, we partnered with the performance team at Ady Watts. Through milestone-based progressions and data-driven monitoring, Lou moved from those first gentle movements back to the gym floor. Today, she isn't just back; she's training at peak capacity with greater resilience and confidence than ever before.
